Addiction Services Central Intake & Navigation - Fraser Health Substance Use Services Access Team (SUSAT)

This team assists people to navigate the substance use service system.
The SUSAT team provides navigation support for addictions services.

Patients may self-refer or physicians can make a referral

SUSAT enables family physicians and primary care clinics to quickly and easily refer people for community-based addictions services such as: counselling group therapy, opioid agonist treatment (OAT), as well as residential treatment, detox and other specialized services.
The team provides:
  • Information
  • Assessment, matching and referral
  • Brief interventions
  • Transitional support among substance use programs
  • Concurrent disorders engagement and referral
  • Rapid re-engagement
  • Support and referral for those who are affected by the substance use of others
Physicians may use the referral form below OR send a standard referral letter to the fax # below.

A referral letter should include: patient contact info, current list of medications, referring clinician's name, and which of the following are of interest to the patient:
  • Withdrawal Management (Detox)
  • Outpatient Substance Use Counselling
  • Residential Substance Use Treatment
  • Opioid Agonist Treatment (Methadone, Suboxone)
  • Other
This team is well suited to people who require added support to engage and remain engaged in services.

SUSAT North and East locations is available 8:30am-6pm Monday to Friday, and the Surrey location is available 8:30am-6pm 7 days a week.
