Seniors' Community Connector - Social Prescribing Program - Denman and Hornby Islands

Provided by Hornby and Denman Community Health Care Society

The Social Prescribing Program supports seniors to access non-clinical, community-based services to prevent or delay frailty by fostering resilience and social support using a comprehensive, strength-based approach.
The community connector offers free one-on-one wellness planning. A dedicated coordinator works with seniors to create personalized plans and facilitates "active linking" to community groups, social activities, and meal programs.

The program is intended for seniors (65+) facing social isolation, loneliness, or barriers to community engagement. This program is free and inclusive of all backgrounds, prioritizing those facing social frailty.
