Mental Health Supported Housing and Residential Program

Assists in providing recovery-oriented services in a stable and supportive living environment for individuals with mental health challenges so that they may work towards the highest level of independence possible.
This program is for:
  • Adults 19+ years of age who are diagnosed with a serious and persistent mental illness.
  • Eligible individuals who are receiving services from their community mental health and substance use team.
Individuals who receive support through the Mental Health Supported Housing and Residential program must be willing participants and actively engage in a program that optimizes their own mental health and wellness.
This program offers a variety of housing supports, from fully supported residential environments to providing services to clients in supported independent living arrangements in their community. These services may be provided by contracted service agencies.
The mental health and substance use team works with individuals and their family to determine which level of housing is most appropriate for each individual.
To access Mental Health Supported Housing and Residential Program services:
  • Some of the mental health supported housing programs require a referral that is completed by health care providers (e.g. family doctor, nurse practitioner) in consultation with the individual’s community mental health and substance use team.
  • Other mental health supported housing programs require an application that is completed by the individual seeking housing with support of their mental health professional.
  • Eligibility for the Mental Health Supported Housing and Residential program is based on an individual’s needs as identified by their community mental health and substance use team.
