Wellness Monitoring Program - Island Health

A program that provides support and guidance as an individual ages or experience health concerns.
Participants in the program partner with a Wellness Mentor to improve or maintain their health, receive guidance in managing their own care needs and to be connected with resources to enhance their quality of life.

Wellness Mentors are Occupational Therapists and Recreation Therapists who use a holistic approach to improving health, which includes physical, emotional and spiritual wellness.

Participants can expect to receive:
  • Guided interviews with patients in their home, community or a clinic setting
  • Home safety checks for high risk and/or frail patients
  • Establish and follow up with individualized action plans to improve health and wellness
  • Receive long term monitoring for patients (by phone or face-to-face) based on their needs
  • Receive referral to appropriate community programs/resources
  • Education and support towards self-management

Eligible participants must be at least 19 years old and have one or more of the following:
  • Stable chronic conditions that do not require urgent interventions
  • Concern(s) regarding their health and wellness (e.g. cognition, chronic disease management, loneliness, …)
  • Diminished social supports and social engagement
  • Diminished physical activity or increased frailty (Clinical Frailty Scale: 4 - Vulnerable*)

Physicians or Nurse Practitioners may call the Physician Connectors (250-519-5282) to place a referral.

Service is available in English.
