Seniors' Community Connector - Social Prescribing Program - Campbell River

Provided by Senior Support North Vancouver Island (SSNVI)

Program supports seniors aged 55 and older who are experiencing social isolation, loneliness, or barriers to community engagement.
The community connector program offers free one-to-one wellness planning for seniors. A dedicated coordinator works with individuals to create personalized plans and facilitates active linking to community groups, social activities, and meal programs.

Assists older adults to age safely in the right place by providing them with the social connections they require, and by building and maintaining partnerships and connections with community-based seniors services in the Campbell River region. This program is free and inclusive of all backgrounds, prioritizing those facing social frailty.

Details: Anybody can refer clients to the Community Connector. Community partners, healthcare professionals, family members, or seniors themselves can refer clients to the Community Connector. People can refer via email, telephone, or in person. A completed referral form is helpful but not required to access support. Once a referral is received, a Community Connector will follow up with the client to within a week of receiving the referral.
